Your Responsibilities as a Patient:
Following Rules and Regulations
You have the responsibility to abide by the rules and regulations of the MCSO facilities and physician’s offices, including observing our NO concealed carry weapon and non-smoking policy.
Providing Information
You have the responsibility, to the fullest extent possible, to completely and accurately provide your healthcare provider with all the information that may affect the care you receive. You are responsible for providing a copy of your advance directive and a list of home medications. You and your family are responsible for reporting any perceived risks that may affect your care and unexpected changes in your condition.
Complying with Treatment Plans and Following Instructions
You are responsible for complying with the treatment plans provided by your MCSO physicians and staff with instructions for your care. If you do not understand the instructions or do not understand what you are expected to do, you should ask your MCSO physician or their staff for an explanation. If you do not follow the instructions given to you, you are responsible for the consequences and for the outcomes of the services or the treatment plan.
Showing Respect and Consideration
You and your visitors are responsible for being considerate of the MCSO facility personnel and property and for being considerate of the needs of other patients by helping to control noise and other disturbances.
Meeting Financial Commitments
You are responsible for promptly meeting any financial obligations agreed to with MCSO, its physicians, facilities and caregivers.

MCSO is dedicated to providing all our patients with the highest quality of care. Developing and maintaining our relationship with you is one of the most important components of delivering the safest possible care.
Learning about your choices in your treatment plan is important for you to make the best possible decisions. We encourage you to ask a family member or friend to assist you in times when you may not be able to fully participate in your care. Never hesitate to bring your concerns to the attention of your healthcare team. We want to know if something does not seem right or you have concerns.
If any of the foregoing “Responsibilities of Patient” is breeched by the patient and your MCSO physician cannot in good conscious move forward with the patient’s care due to such detrimental actions of the patient or their caregiver, then MCSO physician or facility will provide patient and/or its caregiver Notification of Termination of Relationship as medical provider and instruct the patient to seek out new providers or facilities to provide their future or ongoing care. Such notification will be given verbally, if possible and will also be sent in letter form via USPS certified mail with return receipt to the last known address in patient’s medical record.
