Instructions for Fertility Evaluation

• You must abstain from relations four (4) days prior to your appointment.
• A masturbated semen specimen must be brought to the office for analysis at the time of your appointment. This specimen should be no more than one (1) hour old. A container will be provided for your specimen.
• Specimen should be kept at body temperature in route to the office. Keep in a coat pocket or under clothes, close to your body.
