Health Insurance

We strive to provide our patients with excellent family medical care and help them maximize their insurance benefits. Please confirm with your insurance provider to ensure that our practice is listed as an in-network provider under your plan. While we participate with most local insurance plans, we recommend that you verify your coverage before your visit.

Insurance Card: Please bring your insurance card to your appointment so that we can make a copy for our records. Changes in Personal Information: If you have any changes to your insurance, address, or phone number, please inform the receptionist, and we’ll update our system.

Payment Information

Payment at Time of Service: Payment is required at the time of your visit. We accept credit cards, including VISA and MASTERCARD.

Worker’s Compensation and Motor Vehicle Accidents: For visits related to Worker’s Compensation or motor vehicle accidents, payment must be made in cash at the time of the visit. You will need to seek reimbursement from your insurer or through legal settlement.
